Medicina praticada com rigor — e com calma. Medicine practiced with rigor — and without haste.

I graduated in Medicine from the Federal University of Minas Gerais (UFMG) and spent years in emergency medicine before dedicating myself to mental health — an experience that taught me to recognize suffering in its most acute forms and to act precisely when every decision matters.

Why isn’t your depression getting better?

When is it time to seek help?

Psychological suffering rarely arrives all at once. It settles in gradually — in shortened sleep, in work that loses meaning, in irritability that becomes routine.

That is precisely why the person affected is often the last to notice. Those around them see it first: a partner puzzled by the silence, a colleague noticing declining performance, a friend sensing the distance. And there are bodily signs that tend to go unnoticed — unexplained aches, fatigue that rest does not fix, appetite changes, a memory that no longer responds as it used to.

A practical criterion: when symptoms begin to limit what matters to you — relationships, work, enjoyment — for several consecutive weeks, a medical evaluation stops being an overreaction and becomes prudence. It is the same reasoning we apply to any other area of health: no one waits for a heart attack to address their blood pressure.

Seeking help early usually means simpler, shorter treatments with better outcomes. You do not need to hit rock bottom to deserve care — in fact, the best time to reach out is precisely before you get there.

Alta performance tem custo invisívelHigh performance has an invisible cost

Executivos, médicos, advogados, pesquisadores: profissões de alta exigência selecionam pessoas que aprenderam a funcionar apesar do próprio sofrimento. Executives, physicians, lawyers, researchers: demanding professions select for people who have learned to function despite their own suffering.

It is precisely this capacity to endure that delays diagnosis. Chronic anxiety gets rebranded as a "competitive edge"; insomnia becomes "late-night productivity"; exhaustion becomes "a phase." As a result, high performers often arrive at a consultation years after symptoms began — when the accumulated cost is already showing in their decisions, physical health, and relationships.

There is also a legitimate fear: that seeking help will be seen as weakness, or that the information might circulate. That is why medical confidentiality is not a detail of my practice — it is one of its structural conditions. Nothing said in a consultation leaves it.

Proper treatment does not diminish performance — quite the opposite: it restores the clarity, energy, and decision-making capacity that illness silently erodes. Caring for your mind is not stepping out of the game; it is returning to it whole.

O que esperar da primeira consultaWhat to expect from a first visit

Many people postpone a first consultation because they do not know what happens in one. The answer is simple: a long, structured conversation.

There are no invasive procedures and no judgment. We start with what brought you in — in your own words, at your own pace. We then go through your life history, physical health, sleep, current medications, and previous treatments. If you have recent test results or old prescriptions, bring them: they help complete the picture.

At the end, I set aside dedicated time to explain my reasoning: what the diagnostic hypothesis is, what supports it, and what the possible paths forward are — which may or may not include medication. You leave the consultation understanding your own situation in plain language, with a defined plan and knowing exactly what the next steps are.

Nothing is decided without your understanding and consent. The first consultation is not a commitment to a treatment — it is a commitment to clarity.

O primeiro passo é uma conversa. The first step is a conversation.

Por que sua depressão não melhora? Why isn’t your depression getting better?

Observe a expressão “dose e tempo corretos”.

You start losing interest in things, nothing is enjoyable anymore, life feels empty and meaningless. Crying spells begin, your sleep changes, your appetite changes… until you realize you might be depressed. You gather your strength and see a doctor, placing all your hope in that prescription — now, finally, things will get better. The doctor warns you that it takes a few weeks for the effect to kick in, but 3… 4 weeks go by and nothing. The dose goes up and you wait a little longer — still nothing. Frustration sets in and your trust in the treatment is naturally shaken, but this situation is more common than you might think: a study published in The American Journal of Psychiatry estimated that one third of patients did not reach remission after going through 4 levels of treatment, and other studies suggest even worse results.

Many patients ask me this question when they come to see me: why isn’t my depression getting better? Often, the lack of response to previous treatments leads to dissatisfaction and a fear that their case has no solution.

But the path to treating depression is not a straight line; it is a maze that doctor and patient have to walk through together. Some people will feel better without any medication at all: therapy, exercise, meditation, hobbies… and they are back on their feet. Others recover within a few weeks on their first antidepressant. Many others will need to try several medications — 2, 3… — until they find what works best. Sometimes we even need to combine more than one antidepressant, or add medications from another class that have an augmenting effect, such as antipsychotics and mood stabilizers.

Is it really a failure?

Treatment-resistant depression is defined as a lack of response (that is, at least a 50% reduction in symptoms) to at least 2 adequate medication trials, at the right dose and for the right length of time. Note the phrase “right dose and right length of time.” We cannot say a treatment has failed if the doses used never reached therapeutic levels and were not increased when needed, or if the medication was taken for too short a time. Adherence must also be checked. Was the patient taking it every day, at the right time? Were doses often forgotten? That greatly reduces the chance of success.

Patience is needed

The road to recovery, unfortunately, takes time. Each new medication needs a few weeks to start working, so we cannot know whether it has failed without waiting. That is why, for some people, the process can take longer — but there are ways forward! If medications alone are not enough, we can turn to older but highly effective treatments, such as monoamine oxidase inhibitors. We do not prescribe them first because they change many things in the brain and require several adjustments due to side effects, including dietary restrictions. In addition, there are interventional psychiatry treatments, such as esketamine infusion sessions, transcranial magnetic stimulation and electroconvulsive therapy (ECT), which can help a great deal in refractory cases.

Many patients with severe, treatment-resistant depression have improved after an interventional procedure. ECT, for example, is among the most effective treatments for severe depression, with results superior to those of medication in comparative studies. Of course, these steps can be frightening. Our image of electroconvulsive therapy is still shaped by the last century, when it was almost an instrument of torture. Today, performed under anesthesia, it is a safe, painless procedure with excellent results.

So depression that does not improve is not a sign that you will never get better — it is a sign that you need to be with a good professional who can use every available tool. Don’t blame your doctor if it is taking a while; unfortunately, some cases require patience, but with the resources of modern medicine, we have a lot to offer!

It is also worth remembering that treating depression is not only about medication and procedures. Psychotherapy is fundamentally important and works very well alongside medical treatment. A psychiatrist and a psychologist working together gain a real superpower to help patients. Beyond that, exercise, bonds with family and friends, and even spirituality (if that is something that speaks to you) can help along the way.

When the diagnosis is wrong

This is a common trap. In some cases, the patient does not respond to treatment because they are being treated for the wrong thing! We may have someone with borderline personality disorder, marked by emotional instability, which can mimic a depressed mood. Or a patient with bipolar disorder, whose mood stabilization requires other types of medication. A patient with the negative symptoms of schizophrenia (social withdrawal, speaking less and in an impoverished way, loss of interest in things) can also be misleading. That is why successful treatment begins long before medication — it begins with the diagnosis. Marked resistance to multiple interventions justifies revisiting the history, talking to the family and investigating whether the condition is actually a different one that would respond to a different treatment. Once again, being with a professional you trust makes all the difference in this process.

Medical causes

Another trap is that depression is not always primary — that is, it does not always stem from an essentially psychiatric issue. Medical conditions such as hypothyroidism, vitamin B12 deficiency, anemia, disorders of cortisol production, dementias and many others can present with depressive symptoms, and to improve them we need to treat the underlying disease. That is why, when investigating depression, we do need lab tests!

Ultrarapid metabolizers

Finally, some patients are genetically programmed to metabolize certain medications very quickly. This means that, for these people, some antidepressants would be broken down and eliminated so fast that they would not stay in circulation long enough to work. These patients may need medications that are not processed by these “super enzymes.” This is where pharmacogenetic testing is starting to emerge in psychiatry, looking for DNA markers of this overactive metabolism. These tests are expensive and not definitive, so we do not order them routinely, but they can be one more tool in cases where many treatments have already been tried without success. An alternative is to measure blood levels of the medication and its metabolites.

In every case, a good doctor will evaluate each of these questions (and others) in order to choose the best treatment and make the necessary changes until full remission of symptoms — the ultimate goal — is achieved. This path may involve several medications, investigations and therapies, but what matters is not to be discouraged by setbacks and to know that there are many paths and resources for treating patients who are resistant to treatment — always remembering that patience is needed.

Quando a ansiedade precisa de tratamento? When does anxiety need treatment?

A ansiedade crônica

Anxiety is a natural human reaction. We no longer fight lions in the jungle, but the mechanisms that anticipate danger are still present in our brains — only the threats have changed: the deadline for a report, an important meeting that will decide our professional future, a presentation many people will see, an exam, bills to pay… the lions of the 21st century may not kill, but they are just as frightening.

Our baseline, everyday level of anxiety can rise because of all this, but there is a point at which it stops being physiological and becomes a disorder that needs care. In this article I will cover two common forms of anxiety disorder: panic disorder and generalized anxiety disorder (but there are many others!).

Anxiety has its value

There is an evolutionary reason why we feel anxiety. It makes us more alert, more switched on, and improves our performance up to a point. In a way, it prepares us to respond to a crucial moment. But when the harm it causes outweighs the benefits, we need to take a careful look. Within anxiety disorders, there are acute, intense manifestations, and drawn-out, continuous ones.

Panic attacks

One of the most acute and frightening forms of anxiety is the panic attack. A distressing feeling that sets in quickly, within a few seconds or minutes, accompanied by a series of physical changes: your heart races, you can’t catch your breath, there is chest pain or tightness, trembling, tingling, dizziness, nausea, you sweat, your vision blurs, and you may feel distant or disconnected from the world. It is simply awful. Many patients say they feel they are about to die, have a heart attack, faint, go crazy or lose control. It is common to start avoiding places or situations where it might be hard to get out or get help if needed (what we call agoraphobia), or places similar to where previous attacks happened. Having a single panic attack, triggered by an intense emotional event such as very bad news, does not necessarily mean you have a problem; but when we enter the territory of panic disorder — characterized by unexpected, recurrent attacks followed by persistent worry about new attacks or changes in behavior — professional help is needed. It is also worth remembering that attacks sometimes have a medical cause, as in hyperthyroidism, substance use (caffeine, stimulants, cannabis), or withdrawal from alcohol or benzodiazepines. With a first attack, especially with chest pain or shortness of breath, it is essential to seek medical care to rule out cardiac causes. A good professional will assess all of these factors.

Chronic anxiety

Sometimes anxiety does not come in intense attacks, but shows up as a way of existing in which multiple worries, often seen by others as excessive, come to dominate most of one’s thoughts. When your mind always catastrophizes and lives in constant fear that the worst possible situation will happen at any moment, when you always expect the worst outcome, however unlikely, for every action, we may be looking at generalized anxiety disorder. These patients live in constant tension, sleep poorly, cannot relax, feel tired all the time, and life feels like an endless war. Academic and work performance may decline, and relationships may suffer, with greater irritability and lower frustration tolerance. Generalized anxiety disorder is also treatable, and no one needs — or should have — to live like this!

Anyone can be affected

Anxiety disorders are not weakness! An international study found that 9% of the Brazilian population lived with this diagnosis in 2021. When we talk about lifetime prevalence — that is, the percentage of the population that will have an anxiety disorder at some point in life — one study found a figure of 34% in the United States. One in three! And all kinds of people are affected: men, women, adults, older adults, executives, teachers, manual workers. Anxiety does not choose a group, but there are protective factors that reduce the risk, such as family and financial security, regular exercise and even a more optimistic personality. Genetics also plays a role in determining who is more likely to be anxious.

There is a way out!

Treatment of anxiety disorders rests on several pillars: therapy (such as cognitive-behavioral therapy), medication (such as selective serotonin reuptake inhibitors) and lifestyle changes (habits like exercise and meditation). No one needs to suffer from this — there are many science-based resources available.
